1
You report on the last dose.
Loading prescribed like medication: the right movement, at the right dose, progressed every visit by a Doctor of Physical Therapy. This is the part that makes the fix hold.
What It Is
Therapeutic exercise is movement prescribed the way a medication is prescribed: a specific exercise, at a specific load, for a specific number of repetitions, aimed at the tissue your evaluation identified. It is not a workout, and it is not the photocopied sheet you were handed somewhere else. The dose is the treatment, and the dose changes as you do.
The mechanism is straightforward. Tissue adapts to what it is asked to do. A tendon that has been avoided for six months gets less tolerant, not more, and only graded tension turns that around. Muscle that has gone quiet next to a painful joint needs enough load to actually signal a change, which is usually more than people expect and always more than a resistance band on its own. Movement patterns need repetition in the position you actually use them, not on a mat with your eyes closed.
All four of our treating clinicians hold a Doctor of Physical Therapy degree, and that is what stands behind the dosing decisions: how much, how often, in what range, and what has to be true before the next step is added. Hands-on treatment buys you a window of easier movement. This is the work that keeps the window open after you stop coming.
Coached, dosed, and progressed
What It Helps
Anything where the fix has to survive contact with your ordinary week: a job, a staircase, a garden, a season. A sample of where it does the most work:
Strength lost during a painful stretch does not come back on its own once the pain settles. It stays gone quietly, and the first thing that asks for it, a heavy suitcase, a long walk, a weekend of yard work, is usually the thing that starts the whole cycle again.
What A Session Looks Like
1
You report on the last dose.
2
We coach it at close range.
3
You rehearse the home program.
It starts with the report: what the last dose felt like that evening and the next morning. That answer sets today. Then the exercise itself, taught and watched at close range, with your doctor changing the load, the range, or the tempo in real time rather than writing it down and hoping.
You will do fewer exercises than you expect and each one will be harder to argue with. Three things done at the right dose beat ten things done vaguely, every time. Before you leave, the home program is rehearsed in front of us so nothing is a guess in your kitchen at nine at night, and you know exactly what earns the next progression.
Each time we met she would teach me something new. She is great at individualizing exercises.
How It Fits Your Plan
Calm it down.
Small, tolerable doses that quiet the tissue instead of provoking it. Even here, doing nothing is rarely the answer.
Restore what’s stuck.
Loading through the range hands-on work just opened, so the new motion has a reason to stay.
Build strength you trust.
The heaviest phase for this service: real load, honest progression, and side-to-side numbers you have to clear.
Make it stay fixed.
The short keep-it-gone program, sized to your real week and rehearsed with us. Then we graduate you.
Therapeutic exercise fits anyone whose problem keeps coming back after it settles, anyone rebuilding after an injury or a procedure, and anyone who has been told to strengthen it without being told how much, how often, or how to know it is working. It also fits the person who is nervous about loading a joint that has hurt for a long time, which is exactly the situation supervision is for.
If you have a drawer with three printed exercise sheets from three different clinics in it, you are not non-compliant. You were handed a photocopy instead of a dose, and nobody ever checked whether it was working.
Pairs Well With
Therapeutic exercise is rarely the whole plan on its own. It is the half of the plan that outlives your last visit.
Questions, Answered
A workout is dosed to your fitness. Therapeutic exercise is dosed to your injury. The movement, the load, the tempo, and the range are chosen from what your evaluation found, aimed at the tissue that is actually behind the problem, and changed as soon as it stops being the right dose. It is prescribed and supervised by a Doctor of Physical Therapy, which is also why it can be run safely on a joint that a gym program would flare.
No. A ten-item handout is a polite way of guaranteeing nothing gets done. You leave with a small number of things that matter, sized to the week you actually have, and each one gets checked and progressed at your next visit. If something is not fitting into your life, say so and we redesign it, because an exercise you skip has no effect at all.
Working muscle should feel like work. A sharp pain, a joint that swells afterward, or soreness that is still there two days later means the dose was wrong, not that you are weak. Tell your doctor what the exercise felt like that evening and the next morning; that report is how the next dose gets set.
No. Your program is built around what you have, whether that is a doorway, a chair, a set of bands, or a full gym. Everything you are asked to do is taught and practiced in the clinic with your doctor watching first, so you are never guessing about form once you are on your own.
